    Background:

    • Granulocytic sarcoma, also known as chloroma, is a rare tumor mass of immature myeloid cells.
    • It typically occurs in patients with acute myeloid leukemia but can precede or follow the diagnosis.
    • Tumors arising within the urinary system are exceptionally rare.

    Observation:

    • A patient with a history of leukemia presented with unilateral obstructive nephropathy.
    • Imaging revealed a mass within the renal pelvis causing obstruction.
    • The mass was diagnosed as a granulocytic sarcoma.

    Findings:

    • This case represents the first reported instance of granulocytic sarcoma arising within the renal pelvis.
    • The tumor occurred during a period of induced remission of the patient's leukemia.
