Radiological Investigation III: Pulmonary Angiogram and PET Scan

722
Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...

Area of Science:

  • Oncology
  • Radiation Oncology
  • Thoracic Surgery

Background:

  • Thymomas are rare thoracic malignancies with a risk of post-surgical relapse.
  • The role and modalities of post-operative radiotherapy (PORT) for thymoma remain debated.
  • The RADIORYTHMIC trial is a phase 3 study investigating PORT in resected thymoma.

Purpose of the Study:

  • To establish expert consensus recommendations for post-operative radiotherapy in thymoma.
  • To harmonize practices for PORT in completely resected thymoma (Masaoka-Koga stage IIb/III).

Main Methods:

  • A DELPHI method was employed, involving 24 national experts and 115 questions.
  • Key areas addressed included imaging, target volume definition, dose constraints, dose/fractionation, and follow-up.
  • Consensus was defined as ≥80% agreement among experts.

Main Results:

  • Recommendations include preoperative contrast-enhanced CT, 4D CT-based planning or breath-hold techniques, and image fusion.
  • Specific cardiac substructures (coronary arteries) should be delineated.
  • Recommended treatment involves rotational RCMI/VMAT to 50 Gy in 1.8–2 Gy fractions, with cardiac evaluation for high-risk patients.

Conclusions:

  • This study presents the first expert consensus on PORT for thymoma.
  • Implementation of these recommendations is expected to standardize PORT practices.
  • Harmonized PORT strategies may improve treatment efficacy and patient outcomes.
