Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Summary
This summary is machine-generated.

Rituximab plus bendamustine (R-B) followed by rituximab maintenance therapy significantly improves progression-free survival in advanced follicular lymphoma (FL) compared to R-CHOP. This R-B regimen offers a superior frontline treatment option for FL patients.

Area of Science:

  • Hematology
  • Oncology
  • Clinical Trials

Background:

  • Rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP) and R-bendamustine (R-B) are standard first-line treatments for advanced follicular lymphoma (FL).
  • Rituximab maintenance after R-CHOP improves outcomes, but its efficacy after R-B is less understood.

Purpose of the Study:

  • To compare the efficacy and safety of R-B versus R-CHOP followed by rituximab maintenance in untreated advanced-stage FL.
  • To evaluate progression-free survival (PFS) and overall survival (OS) between the two treatment arms.

Main Methods:

  • Retrospective analysis of 476 FL patients from 17 centers receiving R-based regimens with rituximab maintenance.
  • Comparison of complete response rates, relapse frequency, cytopenias, and adverse events during induction and maintenance phases.

Main Results:

  • R-B showed a higher complete response rate and lower relapse frequency compared to R-CHOP.
  • R-CHOP was associated with more frequent induction cytopenias and use of colony-stimulating factors.
  • R-B maintenance led to more neutropenia and infectious toxicity, but did not impact overall mortality.

Conclusions:

  • R-bendamustine followed by rituximab maintenance provides significantly longer PFS than R-CHOP for previously untreated advanced FL.
  • Older patients benefit from R-B without increased toxicity.
  • While R-B maintenance has more adverse events, it does not negatively affect survival outcomes.
