Summary

Gadoxetic acid-enhanced MRI shows intrahepatic mass-forming cholangiocarcinoma (IMCC) subtypes. Hypointense IMCCs suggest aggressiveness with more metastases, but survival outcomes are similar after resection.

Background:

  • Intrahepatic mass-forming cholangiocarcinoma (IMCC) is a challenging liver cancer.
  • Gadoxetic acid (Gd-EOB)-enhanced MRI offers insights into IMCC behavior.
  • Hepatobiliary phase (HBP) enhancement patterns may predict prognosis.

Purpose of the Study:

  • To evaluate the prognostic significance of IMCC enhancement patterns during the HBP on Gd-EOB-enhanced MRI.
  • To correlate HBP features with tumor aggressiveness and patient survival.

Main Methods:

  • Retrospective analysis of 66 IMCC patients with preoperative Gd-EOB-enhanced MRI.
  • Classification of lesions into hypointense and significantly-retaining subtypes based on Gd-EOB retention.
  • Comparison of clinical, radiological, and survival data (RFS, OS).

Main Results:

  • 73% of IMCCs were hypointense, 29% were significantly-retaining.
  • Hypointense IMCCs showed more intrahepatic metastases and infiltrative growth.
  • No significant difference in recurrence-free survival (RFS) or overall survival (OS) between subtypes.
  • Lymph node metastasis independently predicted poor RFS.

Conclusions:

  • Gd-EOB-enhanced MRI identifies hypointense and signal-retaining IMCC subtypes in the HBP.
  • The hypointense subtype indicates potential tumor aggressiveness with increased intrahepatic metastasis.
  • Despite aggressive features, IMCC subtype did not significantly impact survival post-resection.
