Summary

This report details the successful management of a rare penile extraosseous osteosarcoma (EOS) in a 67-year-old male. Surgical intervention and lymph node dissection led to a favorable outcome with no signs of recurrence.

Background:

Penile extraosseous osteosarcoma (EOS) is an exceptionally rare malignancy, with limited documented cases.
This study addresses a unique presentation of a painful penile mass in a 67-year-old male.

Observation:

The patient presented with a two-month history of a painful distal penile mass and bilateral inguinal lymphadenopathy.
Initial imaging and blood work were followed by a partial penectomy.
Histological analysis confirmed high-grade extra-skeletal osteosarcoma with osteoid production and minimal tumor necrosis.

Findings:

The patient underwent successful partial penectomy and subsequent bilateral robotic inguinal lymph node dissection.
Pathology reports confirmed all examined lymph nodes were negative for malignancy.

At five months post-surgery, the patient showed no evidence of disease recurrence.

Implications:

This case highlights the importance of considering rare diagnoses like penile EOS in the differential of penile masses.
Successful surgical management, including lymph node dissection, can lead to favorable outcomes for penile EOS.
Further research into the optimal management strategies for this rare cancer is warranted.
