Integrated Healthcare System01:20

Integrated Healthcare System

1.6K
An integrated healthcare system (IHS) is a set of organizations that provides for or arranges to provide coordinated and continuous service to a defined population. The IHS takes responsibility for that particular population's health status and outcome, both clinically and fiscally. An integrated healthcare system is a well-organized, well-coordinated, and collaborative network. The integrated delivery system is a network that connects different healthcare providers to deliver organized,...

Methods of Documentation V: CBE

900
Charting by Exception, or CBE, is a method of documentation used in healthcare, particularly in nursing, that focuses on documenting only significant or abnormal findings rather than recording every detail. This approach aims to streamline the documentation process, improve efficiency, and ensure that healthcare providers can quickly identify deviations from normalcy in patient assessments.
In CBE, healthcare professionals establish predefined standards of practice that define what constitutes...

The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Methods Of Healthcare Delivery System

3.3K
At the different levels of the healthcare system, we see varying methods of healthcare used. These methods include managed care systems, case management, and primary healthcare.
Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is...

Area of Science:

  • Healthcare Management
  • Health Services Research
  • Integrated Care Models

Background:

  • The global healthcare landscape is shifting towards value-based healthcare (VBHC).
  • Integrated care is a key component of VBHC, yet practical guidance for its implementation is scarce.
  • This review addresses the definition, effects, and implementation challenges of value-based integrated care (VBIC).

Approach:

  • A systematic literature search was conducted across Embase, Medline ALL, Web of Science, and Cochrane databases until January 2022.
  • Empirical studies evaluating integrated care within VBHC and non-empirical studies defining VBIC or its implementation factors were included.
  • Qualitative analysis, guided by the Rainbow Model of Integrated Care (RMIC), was performed, with study quality assessed by the Mixed Methods Appraisal Tool (MMAT).

Key Points:

  • Only one article provided a definition for VBIC, indicating a lack of conceptual clarity.
  • Eleven studies reported predominantly positive effects of VBIC on clinical outcomes, patient-reported outcomes, and healthcare utilization.
  • Nineteen studies identified facilitators and barriers, with reimbursement and IT infrastructure emerging as critical factors.

Conclusions:

  • The definition of VBIC requires further refinement.
  • While VBIC shows potential benefits, effect evaluations are complicated by multicomponent interventions and generalizability issues.
  • Successful VBIC implementation necessitates strategic investments in IT infrastructure and innovative reimbursement strategies.
