    Background:

    • Tricyclic antidepressants (TCAs) are widely prescribed for various psychiatric conditions.
    • Common side effects of TCAs can impact patient adherence and quality of life.
    • Gastrointestinal and metabolic side effects, such as appetite changes, require further investigation.

    Purpose of the Study:

    • To determine the prevalence of excessive appetite and craving for sweets in patients on TCAs.
    • To examine the relationship between these food craving side effects and medication variables, patient characteristics, and clinical outcomes.
    • To highlight the clinical significance of managing these side effects, particularly concerning weight gain.

    Main Methods:

    • A cross-sectional study involving 93 outpatients receiving TCAs for at least one month.
    • Data collection included patient-reported medication side effects (appetite, sweet cravings), medication details (type, dose), patient demographics, diagnoses, weight changes, and clinical improvement.
    • Statistical analysis using Pearson correlation (r) was employed to assess relationships between variables.

    Main Results:

    • 38% of patients reported excessive appetite, and 34% reported craving sweets; 48% experienced at least one of these side effects.
    • These food craving side effects were significantly associated with weight gain (P < .001) and higher TCA dosage.
    • No significant relationship was found between food craving side effects and clinical improvement, although overall improvement was moderate-to-good.

    Conclusions:

    • Excessive appetite and sweet cravings are common side effects of tricyclic antidepressants, impacting nearly half of patients studied.
    • These adverse effects are strongly correlated with increased weight gain and higher medication doses, posing potential health risks.
    • Future research and clinical strategies should focus on mitigating these food-related side effects to improve patient outcomes and long-term adherence to TCA therapy.
