Background:

  • Lung cancer is a leading cause of cancer death globally.
  • Low-dose computed tomography (LDCT) screening reduces lung cancer mortality in high-risk individuals.
  • Lung cancer screening adoption outside the US has been limited.

Purpose of the Study:

  • To evaluate the implementation and outcomes of a risk-based lung cancer screening program in Ontario.
  • To assess the lung cancer detection rate, early-stage diagnosis proportion, and safety of the screening pilot.

Main Methods:

  • Recruited 7,768 high-risk individuals using the PLCOm2012noRace model.
  • Successfully screened, retained, and followed up 4,451 participants.
  • Collected data on lung cancer detection, stage at diagnosis, harms, and adherence.

Main Results:

  • Lung cancer detection rate was 2.4%, with 79.2% diagnosed at an early stage.
  • Sensitivity for detecting lung cancer was ≥95.3%; specificity was 95.5%.
  • Serious harms were infrequent, and adherence to screening protocols was high (>85%).

  • The Ontario Lung Cancer Screening Pilot successfully implemented a risk-based organized lung screening program.
