Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.

Background:

  • Castration-resistant prostate cancer (CRPC) management benefits from radiographic assessment.
  • This study retrospectively analyzed radiographic changes from metastatic hormone-sensitive prostate cancer (mHSPC) to CRPC diagnosis.
  • The impact of these changes on prognosis post-CRPC was explored.

Purpose of the Study:

  • To determine if radiographic progression at CRPC diagnosis impacts overall survival (OS).
  • To evaluate the prognostic value of radiographic assessment in mHSPC patients progressing to CRPC.

Main Methods:

  • Retrospective analysis of 98 men diagnosed with CRPC.
  • Radiographic examinations (bone scans, CT) at CRPC diagnosis were reviewed.
  • Patients were grouped into radiographic progressive disease (rPD) and non-rPD based on progression.

Main Results:

  • Median OS post-CRPC was significantly shorter in the rPD group (32 months) versus the non-rPD group (not reached, P = .0124).
  • Multivariate analysis revealed radiographic progression independently predicted shorter OS post-CRPC (HR = 3.14, P = .019).
  • Shorter time to CRPC was also associated with reduced OS post-CRPC.

Conclusions:

  • Radiographic progression at CRPC diagnosis is an independent predictor of shorter overall survival in mHSPC patients.
  • Radiographic assessment at CRPC diagnosis is crucial for effective patient management.
  • Early identification of radiographic progression can inform treatment strategies for CRPC.
