Background:

  • Multiple sclerosis (MS) causes multifaceted gait impairments affecting mobility, cognition, and fall risk.
  • These factors are often studied in isolation, limiting understanding of their combined impact on real-world walking.

Purpose of the Study:

  • To investigate how mobility, cognition, and fear of falling contribute to real-world gait quality and quantity in individuals with MS.

Main Methods:

  • Twenty individuals with MS completed cognitive tests (PASAT, SDMT, Stroop, SRT), the Falls Efficacy Scale - International (FES-I), and the Patient Determined Disease Steps (PDDS).
  • Participants wore an inertial sensor for three days to collect real-world gait data (speed, stride regularity, time walking, bouts).

Main Results:

  • Significant correlations were found between gait speed/stride regularity and SDMT, FES-I, and PDDS.
  • Cognition (SDMT) and walking impairment (PDDS) explained 63% of gait speed variance and 69% of stride regularity variance.
  • No significant correlations were found for gait quantity with cognition or fear of falling after adjusting for age and sex.

Conclusions:

  • Cognition, walking impairment, and fear of falling play crucial roles in real-world gait quality for individuals with MS.
  • These findings can guide the development of targeted interventions to enhance walking in MS.
  • Further research is needed to identify factors influencing gait quantity in this population.
