Cholecystitis01:20

Cholecystitis

Cholecystitis is inflammation of the gallbladder, most commonly caused by obstruction of the cystic duct. This blockage prevents bile from draining, leading to gallbladder distension, inflammation, and potentially serious complications. This condition may present acutely or chronically and can happen with or without gallstones.EtiologyAbout 95% of cholecystitis cases are calculous, caused by gallstones blocking the cystic duct, leading to bile accumulation and inflammation of the gallbladder...

Area of Science:

  • Oncology
  • Surgical Pathology

Background:

  • Gallbladder carcinosarcoma (GBC) is exceptionally rare, with fewer than 100 reported cases.
  • This malignancy presents significant diagnostic challenges and carries a very poor prognosis.

Observation:

  • A 65-year-old male presented with abdominal pain, initially misdiagnosed as a liver abscess.
  • Imaging revealed a mass in the liver and gallbladder fossa; initial abscess drainage failed.
  • Subsequent hepatectomy and cholecystectomy confirmed GBC with biphasic components.

Findings:

  • The patient experienced rapid tumor recurrence one month post-surgery, leading to duodenal compression, obstruction, and bleeding.
  • Despite interventions, the patient succumbed to gastrointestinal hemorrhage and hypovolemic shock.

Implications:

  • This case underscores the difficulty in preoperative diagnosis of GBC.
  • Rapid recurrence and poor outcomes highlight the aggressive nature of this rare tumor.
  • Increased clinical vigilance is crucial for timely and accurate diagnosis of gallbladder carcinosarcoma.
