Endoscopic Procedures III: Video Capsule Endoscopy01:28

Endoscopic Procedures III: Video Capsule Endoscopy

138
Capsule endoscopy, or wireless or video capsule endoscopy, is a diagnostic procedure for examining the entire gastrointestinal tract. Patients swallow a capsule about the size of a vitamin tablet. The capsule is equipped with a transmitter, a battery, an LED light source, and a color video camera to capture images throughout the gastrointestinal tract. This procedure is particularly useful for diagnosing conditions such as Crohn's disease, ulcerative colitis, tumors, polyps, ulcers,...

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

78
Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Endoscopic Procedures V: ERCP

202
Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Endoscopic Procedures II: Colonoscopy

90
The colon, or large intestine, is the final segment of the digestive system. Its primary functions include absorbing water and vitamins produced by gut bacteria and transforming waste from liquid to solid to form stool. In adults, the large intestine is approximately 5 feet long and consists of four main sections:

Endoscopic Procedures I: Esophagogastroduodenoscopy

109
An Esophagogastroduodenoscopy (EGD) is a diagnostic procedure in which an endoscopist uses a flexible, lighted endoscope to visualize the upper gastrointestinal (GI) tract. The procedure includes visualizing the oropharynx, esophagus, stomach, and the first part of the small intestine, the duodenum.
During an EGD, the endoscope can be used to:

Imaging Studies III: Gastrointestinal Motility Studies and Virtual Colonoscopy

83
This lesson explores three gastrointestinal imaging techniques: radionuclide testing, colonic transit studies, and virtual colonoscopy.
Radionuclide Testing
Radionuclide testing is a sophisticated medical technique for assessing gastrointestinal motility. It focuses on gastric emptying and colonic transit time. Radioactive markers track the movement of food through the digestive system, providing insights into gastrointestinal disorders.
In gastric emptying studies, a meal's liquid and...

Summary

Artificial intelligence (AI) reading significantly improved diagnostic yield for small bowel bleeding lesions compared to standard methods. AI-assisted capsule endoscopy reading is faster and more accurate for detecting these lesions.

Area of Science:

  • Gastroenterology
  • Medical Imaging
  • Artificial Intelligence

Background:

  • Capsule endoscopy reading is time-intensive and requires sustained reader attention.
  • Deep convolutional neural networks show potential to exceed human performance in identifying capsule endoscopy findings.
  • Reducing reading time and improving accuracy are key goals in capsule endoscopy analysis.

Purpose of the Study:

  • To assess the non-inferiority of artificial intelligence (AI)-assisted capsule endoscopy reading versus standard reading for small bowel bleeding lesions.
  • To evaluate the diagnostic yield of AI-assisted reading for P1 and P2 small bowel bleeding lesions.
  • To compare the mean reading time between AI-assisted and standard capsule endoscopy reading modalities.

Main Methods:

  • Prospective enrollment of 133 patients (aged ≥18) with suspected small bowel bleeding across 14 European centers.
  • Patients underwent capsule endoscopy using the Navicam SB system with an integrated AI (ProScan) for lesion detection.
  • Blinded reading was performed in standard mode, followed by AI-assisted reading where AI performed the initial automated read.

Main Results:

  • AI-assisted reading demonstrated non-inferiority and superiority (73.7% vs 62.4%) in the per-patient diagnostic yield of P1 and P2 lesions.
  • The mean reading time was significantly reduced with AI-assisted reading (3.8 min) compared to standard reading (33.7 min).
  • 133 patients were analyzed, with 112 completing the capsule endoscopy procedure.

Conclusions:

  • AI-assisted capsule endoscopy reading offers a more accurate detection of small bowel bleeding lesions.
  • AI significantly reduces the time required for capsule endoscopy interpretation.
  • AI-assisted reading presents a promising advancement for diagnosing small bowel bleeding.
