Area of Science:

  • Neurosurgery
  • Neurology
  • Developmental Biology

Background:

  • Hypothalamic hamartomas (HH) are rare congenital malformations.
  • HH often lead to refractory epilepsy, particularly gelastic seizures.
  • Surgical intervention is a key treatment for drug-resistant epilepsy and endocrinopathies associated with HH.

Purpose of the Study:

  • To detail the full-endoscopic surgical technique for hypothalamic hamartoma resection.
  • To evaluate the safety and efficacy of this minimally invasive approach.

Main Methods:

  • Utilized a 30° rigid endoscope with specialized instruments (microforceps, monopolar electrocautery).
  • Employed intraoperative ultrasonography (USG) for guidance.
  • Described the procedural steps and required equipment.

Main Results:

  • The full-endoscopic approach is technically feasible and reproducible after a learning curve.
  • Shorter operative times compared to open surgery.
  • Demonstrated reduced blood loss, low surgical site pain, and facilitated early patient mobilization.

Conclusions:

  • Full-endoscopic resection of hypothalamic hamartomas is a safe and effective minimally invasive technique.
  • Achieved favorable seizure control and endocrinological outcomes.
  • Offers significant advantages over traditional open surgery, including faster recovery.
