Intracranial atherosclerotic disease (ICAD) is common in acute basilar artery occlusion (BAO). Despite differences in occlusion location and treatment timing, ICAD did not impact 90-day outcomes for patients undergoing endovascular therapy.

- No significant differences were observed in good functional outcome (45.5% vs. 44.8%), symptomatic hemorrhage (4.5% vs. 8.3%), or mortality (42% vs. 50%) between groups.
Conclusions:
- ICAD is a frequent cause of BAO, often associated with proximal occlusions and dyslipidemia.
- Despite differing characteristics and treatment nuances, ICAD did not significantly affect 90-day functional outcomes, hemorrhage rates, or mortality in BAO patients treated with endovascular therapy.
