Background:

Robotic assistance is increasingly adopted in general surgery.
Previous research confirms the safety and efficacy of robotic surgery.
Limited data exists on national temporal and regional trends in robotic surgery implementation.

Purpose of the Study:

To evaluate national trends in robotic surgery utilization.
To compare trends across different surgical specialties and organ systems.
To analyze trends based on hospital characteristics and geographic regions.

Main Methods:

Retrospective population-based study using the National Inpatient Database (NIS) from 2009-2014.
Identification of adult robotic-assisted procedures using ICD-9 codes (17.41-17.49).
Analysis of trends by hospital size, teaching status, US geographic region, and procedure type.

Main Results:

Overall robotic surgery utilization increased by 64% between 2009 and 2014.
Biliary (2984%) and hernia (1376%) procedures showed the largest utilization increases.
Medium-sized hospitals (41%) and rural non-teaching hospitals (274%) demonstrated the largest increases in robotic utilization, while large urban teaching hospitals saw decreases in many procedures.

Conclusions:

Robotic surgery utilization has significantly increased nationwide, with notable adoption in smaller and rural healthcare settings.
A shift in robotic surgery implementation is observed, with decreased utilization in large urban teaching hospitals and increased use in community settings.
Further research is needed to understand the drivers of increased robotic surgery adoption in smaller institutions.
