Background:

Prolonged postoperative ileus (PPOI) presents a significant clinical challenge and healthcare burden following colorectal surgery.
Existing risk prediction algorithms aim to identify patients at high risk for PPOI, but their comparative performance is unclear.

Approach:

A systematic literature search identified 11 studies involving 87,9549 participants that developed or validated PPOI risk prediction algorithms.
Data on study design, patient and operative characteristics, PPOI definitions, and algorithm performance were extracted and appraised using the PROBAST tool.

Key Points:

Commonly included variables were sex, age, and surgical approach, with reported Area Under the Curve (AUC) ranging from 0.68-0.78.
Significant heterogeneity existed in PPOI definitions across studies, and only three models underwent validation.
Most studies were retrospective and single-center, limiting generalizability.

Conclusions:

Current PPOI risk prediction algorithms demonstrate moderate predictive capability but suffer from a lack of external validation.
Future research should prioritize standardized PPOI definitions, comprehensive performance reporting, and rigorous internal and external validation methodologies.
