Area of Science:

  • Neurology
  • Integrative Medicine
  • Clinical Research

Background:

  • The efficacy of acupuncture for promoting wakefulness in poststroke coma patients is debated.
  • Acupuncture is a common clinical practice with potential neurological applications.

Purpose of the Study:

  • To systematically evaluate the effectiveness of acupuncture in treating patients with poststroke coma.
  • To synthesize evidence from randomized controlled trials (RCTs) on acupuncture for poststroke coma.

Main Methods:

  • A comprehensive literature search was conducted across multiple databases up to November 25, 2023.
  • Meta-analysis of 34 RCTs involving 2757 patients was performed using Stata 17 and Review Manager 5.4.
  • Key outcome measures included Glasgow Coma Scale (GCS), National Institute of Health Stroke Scale (NIHSS), awakening ratio, and clinical effectiveness.

Main Results:

  • Acupuncture significantly improved GCS scores (WMD = 1.78) and NIHSS scores (WMD = -2.84) compared to control groups.
  • Acupuncture combined with routine treatment demonstrated superior awakening ratios (RR = 1.65) and clinical effectiveness (RR = 1.20).
  • Identified methodological limitations included lack of blinding, inconsistent assessments, and varied interventions.

Conclusions:

  • Current evidence suggests acupuncture, as an adjunct to conventional therapy, may effectively treat poststroke coma.
  • High-quality RCTs addressing randomization, blinding, intervention standardization, and long-term follow-up are necessary to validate these findings.
