Background:

  • Transitional cell carcinoma (TCC) of the bladder, especially muscle-invasive and high-risk non-muscle-invasive types, necessitates aggressive treatment.
  • Radical cystectomy with pelvic lymphadenectomy is a standard curative-intent surgical approach for these conditions.

Purpose of the Study:

  • To evaluate the clinical and oncological outcomes of radical cystectomy with pelvic lymphadenectomy.
  • To assess the impact of this procedure on renal function in patients with bladder cancer.

Main Methods:

  • A retrospective analysis of 156 patients who underwent radical cystectomy with pelvic lymphadenectomy between January 2015 and December 2022.
  • Estimated glomerular filtration rate (eGFR) was calculated using the Modification of Diet in Renal Disease (MDRD) formula pre- and post-operatively.

Main Results:

  • Of 132 primary TCC patients, 86.36% had high-grade disease and 53.02% had organ-confined disease. Nodal extension was observed in 56.06% of patients.
  • Overall survival and recurrence-free survival (RFS) at 5 years were 66.67% and 45.45%, respectively. RFS was associated with pathological stage, nodal status, grade, surgical margin, and time from diagnosis.
  • Post-operatively, 34.8% of patients showed improved eGFR, while 65.2% experienced deterioration over a median follow-up of 62 months.

Conclusions:

  • Radical cystectomy with pelvic lymphadenectomy yields favorable overall survival outcomes for bladder cancer.
  • Pre-operative renal function, indicated by eGFR, is a crucial factor influencing long-term post-operative renal function.
