Background:

  • Periprosthetic fractures and implant loosening are known complications of total knee arthroplasty (TKA).
  • Malignant tumors arising around orthopedic implants are exceptionally rare, posing diagnostic and therapeutic challenges.

Observation:

  • A 79-year-old woman experienced a periprosthetic fracture 8 years post-TKA.
  • Radiographs revealed tibial implant loosening and severe osteolysis.
  • A diagnosis of high-grade osteosarcoma surrounding the prosthesis was confirmed.

Findings:

  • The patient underwent a supracondylar femoral amputation.
  • No complications were reported 2 years after the amputation.
  • This case highlights an unusual presentation of osteosarcoma in the context of TKA.

Implications:

  • Vigilance is required for patients with rapidly progressive or aggressive implant loosening and osteolysis post-TKA.
  • Multidisciplinary team guidance is essential for managing these complex cases.
  • Limb salvage or amputation are often necessary treatment modalities for periprosthetic osteosarcoma.
