Background:

  • Vertebral artery dissections (VAD) are a leading cause of stroke in young adults.
  • Cervical spine manipulation (CSM) is a potential trigger for VAD.
  • Prompt diagnosis and management are crucial for patient outcomes.

Observation:

  • A 48-year-old patient presented with ataxia, vertigo, and neck pain after CSM.
  • Computed tomography angiography (CTA) revealed a right extracranial VAD with a string sign.
  • Ultrasound (US) demonstrated high-resistance flow in the affected artery.

Findings:

  • Magnetic resonance imaging (MRI) confirmed a VAD/hematoma and a posterior inferior cerebellar artery (PICA) territory infarct.
  • The dissection involved the V3 segment of the right vertebral artery.
  • The infarct measured 1.4 cm in the right PICA territory.

Implications:

  • Neck pain can indicate either musculoskeletal issues or serious neurovascular conditions like VAD.
  • Healthcare providers must consider neurovascular origins of neck pain before performing CSM.
  • Early recognition and differentiation of neck pain causes are vital to prevent VAD-related strokes.
