Background:

Purpose of the Study:

  • To compare the weight loss efficacy of a wireless feedback system (WFS) alone versus WFS with telephone coaching.
  • To evaluate the effectiveness of different step-up interventions for participants who do not respond to initial weight loss treatment.

Main Methods:

  • A noninferiority randomized trial involving 400 adults (BMI 27-45) compared WFS versus WFS plus coaching for 3 months.
  • Participants with suboptimal weight loss were rerandomized to modest or vigorous step-up interventions.
  • Weight change was assessed at 6, 3, and 12 months; noninferiority margin was -2.5 kg.

Main Results:

  • The WFS group achieved a mean 6-month weight loss of -2.8 kg, while the WFS plus coaching group lost -4.8 kg (difference -2.0 kg).
  • The WFS was not noninferior to WFS plus coaching, as the 90% CI for the difference included the -2.5 kg margin.
  • Step-up interventions for nonresponders did not yield significant differences in weight loss.

Conclusions:

  • A wireless feedback system alone is not noninferior to the same system with added telephone coaching for weight loss.
  • Continued research is essential to identify effective weight loss management strategies and personalize interventions for individual needs.
