Endoscopic Studies I: Bronchoscopy and Thoracoscopy01:30

Endoscopic Studies I: Bronchoscopy and Thoracoscopy

192
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due...

Radiological Investigation III: Pulmonary Angiogram and PET Scan

91
Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...

Radiological Investigation II: MRI and Ventilation Perfusion Scan

114
Description
Magnetic Resonance Imaging (MRI) and Ventilation Perfusion Scans are two radiological investigations that offer detailed diagnostic images of the body, particularly lung structures.
MRI
MRI uses magnetic fields and radiofrequency signals to distinguish between normal and abnormal tissues. This technology provides a more detailed diagnostic image than CT scans, enabling it to characterize pulmonary nodules, stage bronchogenic carcinoma, and evaluate inflammatory activity in...

Summary

Electromagnetic navigation bronchoscopy (ENB) shows high specificity for diagnosing peripheral pulmonary nodules (PPNs) under 2 cm. However, diagnostic accuracy decreases for nodules smaller than 1 cm and those with a spiculated sign.

Area of Science:

  • Pulmonology
  • Medical Imaging
  • Diagnostic Procedures

Background:

  • Electromagnetic navigation bronchoscopy (ENB) is recognized for its high sensitivity in diagnosing peripheral pulmonary nodules (PPNs).
  • The diagnostic yield of ENB for smaller PPNs (<2 cm) requires further evaluation.

Purpose of the Study:

  • To evaluate the diagnostic yield of ENB-mediated biopsy for peripheral pulmonary nodules (PPNs) less than 2 cm in diameter.
  • To assess the sensitivity, specificity, positive predictive value, and negative predictive value of ENB for these PPNs.

Main Methods:

  • A retrospective analysis of patients who underwent ENB-mediated biopsy for PPNs <2 cm between May 2015 and February 2020.
  • Final diagnosis was confirmed via pathological examination after surgical resection.

Main Results:

  • The overall diagnostic yield for PPNs <2 cm demonstrated a sensitivity of 53.3% and specificity of 91.7%.
  • Diagnostic accuracy was higher for nodules ≥1 cm (sensitivity 62.5%) compared to those <1 cm (sensitivity 30.8%).
  • The presence of a spiculated sign negatively impacted ENB diagnostic accuracy (p=0.010).

Conclusions:

  • ENB exhibits good specificity and positive predictive value for PPNs <2 cm.
  • Diagnostic reliability is potentially limited to nodules ≥1 cm.
  • Spiculated PPNs may reduce the accuracy of ENB.
