Summary

Background:

  • Current US physical activity guidelines recommend 150 minutes of moderate to vigorous physical activity (MVPA) weekly.
  • A step-based recommendation is lacking due to insufficient evidence.
  • Older adults represent a key demographic for physical activity interventions.

Purpose of the Study:

  • To investigate the association between MVPA time and daily step counts with all-cause mortality and cardiovascular disease (CVD).
  • To compare the effectiveness of MVPA time versus step counts in predicting health outcomes.

Main Methods:

  • A cohort study analyzed data from 14,399 women aged 62 years and older from the Women's Health Study.
  • Physical activity was measured using accelerometers over 7 consecutive days.
  • Cox proportional hazards regression and survival analysis were used to assess associations with mortality and CVD over a median follow-up of 9 years.

Main Results:

  • Higher MVPA time and step counts were associated with reduced all-cause mortality and CVD.
  • Each 1 SD increase in MVPA time and step counts corresponded to a significant reduction in mortality risk (HR 0.82 and 0.74, respectively).
  • Both metrics showed similar predictive ability for all-cause mortality (AUC 0.55 for both).

Conclusions:

  • MVPA time and step counts demonstrate similar associations with all-cause mortality and CVD in older women.
  • Step count goals should be considered for future physical activity guidelines.
  • Incorporating step-based goals can offer flexibility and accommodate personal preferences in activity tracking.
