    • Solitary pulmonary nodules (SPNs) present a diagnostic challenge, often mimicking malignancy.
    • Fibroblast Activation Protein (FAPI) is increasingly targeted in molecular imaging.

    Observation:

    • A case of pulmonary inflammatory pseudotumor demonstrated significantly elevated 68Ga-FAPI uptake.
    • The pseudotumor appeared as a cancer-like solitary pulmonary nodule on imaging.

    Findings:

    • Elevated 68Ga-FAPI uptake was observed in a pulmonary inflammatory pseudotumor.
    • This finding highlights a potential imaging biomarker for non-malignant conditions.

    Implications:

    • Pulmonary inflammatory pseudotumor should be included in the differential diagnosis of SPNs with high 68Ga-FAPI uptake.
    • This expands the understanding of FAPI-targeted imaging beyond oncology.
    • Further research is needed to elucidate the role of FAPI in inflammatory processes.
