    Pulmonary histiocytosis X in four patients presented with lung density changes and cysts. Diagnosis often required open lung biopsy, but steroid treatment normalized T-cell distribution.

    Background:

    • Pulmonary histiocytosis X (PHX) is a rare lung disease.
    • It affects young adults and can present with diverse radiological findings.

    Observation:

    • Radiological findings include reticulo-nodular increases in density and cystic changes, predominantly in the upper lung fields.
    • Pneumothorax and bone lesions are frequently observed in patients with this condition.
    • Diagnosis often necessitates open lung biopsy, even after transbronchial biopsies.

    Findings:

    • Bronchoalveolar lavage in one patient showed a high lymphocyte count (28%), with a predominance of T-suppressor cells over T-helper cells.
    • This T-cell imbalance normalized following corticosteroid therapy.

    Implications:

    • Early identification of T-cell abnormalities via bronchoalveolar lavage may aid in diagnosis and monitoring.
    • Steroid treatment appears effective in restoring normal T-cell distribution in pulmonary histiocytosis X.
    • Further research into the immunological aspects of this condition is warranted.
