    Gallbladder radionuclide uptake occurred in patients undergoing gastrointestinal bleeding scans using technetium-99m labeled red blood cells. This finding may be linked to heme labeling, especially in patients with severe kidney impairment and prior transfusions.

    Background:

    • Technetium-99m (99mTc)-labeled red blood cell (RBC) scintigraphy is a standard imaging technique for detecting gastrointestinal (GI) bleeding.
    • Gallbladder visualization is an uncommon finding in 99mTc-RBC studies.

    Observation:

    • Five patients exhibited unexpected radionuclide activity localized to the gallbladder on delayed imaging after 99mTc-RBC injection for suspected GI bleeding.
    • This observation was noted during routine diagnostic procedures.

    Findings:

    • The mechanism is hypothesized to involve the labeling of heme, a precursor to bilirubin, by 99mTc.
    • All affected patients had a history of blood transfusions.
    • Severe renal impairment was present in four out of five patients, suggesting it as a potential predisposing factor.
