    Background:

    • Immunoglobulin A (IgA) deficiency is the most common primary immunodeficiency.
    • Understanding the associated immune system alterations is crucial for patient management.

    Purpose of the Study:

    • To compare the immune system status of individuals with isolated IgA deficiency to healthy controls.
    • To investigate potential immunoregulatory mechanisms contributing to IgA deficiency.

    Main Methods:

    • Comparative analysis of immune parameters in 23 IgA-deficient subjects and 15 controls.
    • Assays included serum IgM levels, mitogen stimulation indices (Con A, PWM, PHA), T-cell activity (suppressor cells, Leu 3/Leu 2 ratio), chemotactic activity, and neutrophil function (NBT test).

    Main Results:

    • IgA-deficient group showed higher serum IgM and reduced stimulation indices for Con A, PWM, and PHA (significant for PHA).
    • Decreased suppressor T-cell activity, significantly reduced chemotactic activity, lower Leu 3 counts, and a significantly lower Leu 3/Leu 2 ratio were observed.
    • Reduced mean percentage of positive NBT neutrophils indicated impaired neutrophil function.

    Conclusions:

    • The findings suggest an imbalance in immunoregulatory mechanisms in isolated IgA deficiency.
    • These immune dysregulations may play a role in the pathophysiology of IgA deficiency.
