Purpose of the Study:

  • To systematically review and meta-analyze clinical characteristics and comorbidities associated with critical COVID-19 in children and adolescents.
  • To inform risk stratification strategies for pediatric COVID-19 management.

Main Methods:

  • Systematic literature search of Medline and EMBASE up to August 31, 2023.
  • Inclusion of studies reporting outcome data for patients aged ≤21 years with COVID-19.
  • Random-effects models used to estimate pooled odds ratios for critical disease, defined as mechanical ventilation, ICU admission, or death.

Main Results:

  • 136 studies met inclusion criteria; 70 studies (172,165 patients) were included in the meta-analysis.
  • Absolute risk of critical COVID-19 in previously healthy children was 4%.
  • Comorbidities markedly increased critical COVID-19 risk: OR 3.95 for one comorbidity, 9.51 for ≥2 comorbidities. Key risk factors included cardiovascular, neurological, chronic pulmonary (excluding asthma), diabetes, obesity, and immunocompromise.

Conclusions:

  • The absolute risk of critical COVID-19 is low in healthy children and adolescents.
  • The presence of one or more comorbidities significantly elevates the risk of critical COVID-19.
  • Findings underscore the importance of risk stratification for tailoring pediatric COVID-19 treatment and management.
