    Summary

Emergency endoscopic sclerotherapy (EEST) effectively controls active variceal bleeding. This safe and feasible procedure, using absolute alcohol, achieved high success rates in patients with portal hypertension.

    Area of Science:

    • Gastroenterology
    • Endoscopy
    • Hepatology

    Background:

    • Variceal bleeding is a serious complication of portal hypertension.
    • Effective management of active variceal bleeding is critical to prevent mortality.

    Purpose of the Study:

    • To evaluate the safety and efficacy of emergency endoscopic sclerotherapy (EEST) for active variceal bleeding.
    • To assess the feasibility of EEST as a first-line treatment.

    Main Methods:

    • 38 patients with variceal bleeding underwent EEST using absolute alcohol.
    • Technical success, bleeding control, rebleeding episodes, and complications were recorded.
    • Repeated EEST sessions were administered for rebleeding episodes within 24 hours.

    Main Results:

    Related Experiment Videos

    • EEST was technically adequate in 95% of patients, with successful bleeding control in 92%.
    • Overall success rate, including repeated sessions, was 87%.
    • Complications were minor and transient; three deaths occurred, unrelated to the procedure itself.

    Conclusions:

    • EEST is technically feasible and effective for managing active variceal bleeding.
    • The procedure is relatively safe and can be considered a first-line treatment option.
    • Innovations in EEST, including repeated sessions and specific technical aspects, enhance its effectiveness.
