Takotsubo syndrome, a stress-induced cardiomyopathy, can mimic acute coronary syndrome (ACS). This case highlights a rare instance of Takotsubo cardiomyopathy coexisting with obstructive coronary artery disease in a patient with a history of rectal cancer.
