Summary

Comparing sacroiliac joint (SIJ) pain injections, steroid treatments show effectiveness but have higher failure rates than platelet-rich plasma (PRP) injections. Further research is needed to determine the optimal SIJ pain management strategy.

Background:

Sacroiliac joint (SIJ) pain management lacks a definitive injective treatment.
Quantifying the safety and effectiveness of SIJ injective strategies is crucial.

Purpose of the Study:

To systematically review and meta-analyze the safety and efficacy of various injective treatments for SIJ pain.
To compare the outcomes of different injection therapies for SIJ pain.

Main Methods:

Systematic review and meta-analysis of PubMed, Scopus, and Embase databases up to January 2023.
Inclusion of comparative and non-comparative studies, case series on SIJ injections.
Quality assessment using the Downs and Black checklist.

Main Results:

43 papers (2431 patients) analyzed; 63% steroid, 16% PRP injections.
Steroid injection failure rate: 26%; PRP injection failure rate: 14%.
Both steroids and PRP significantly reduced VAS pain scores at mid-term and long-term follow-ups.

Conclusions:

Steroid injections are well-documented for SIJ pain but have a notable failure rate.
Platelet-rich plasma (PRP) shows promising early results for SIJ pain.
Current literature limitations necessitate further studies to establish the optimal SIJ injective treatment.
