    Background:

    • Hepatic cirrhosis is associated with metabolic disturbances.
    • Insulin resistance is a common complication in patients with liver disease.

    Purpose of the Study:

    • To investigate in vivo insulin sensitivity and adipocyte insulin binding and action in patients with hepatic cirrhosis.
    • To compare these parameters with normal control subjects.

    Main Methods:

    • Assessed in vivo insulin sensitivity using euglycemic hyperinsulinemic clamp and glucose-insulin infusion techniques.
    • Measured adipocyte insulin binding and in vitro insulin-stimulated lipogenesis.
    • Analyzed monocyte insulin binding.

    Main Results:

    • Cirrhotic patients showed impaired oral glucose tolerance and in vivo insulin resistance.
    • Adipocytes from cirrhotic patients had significantly lower insulin binding and reduced sensitivity to insulin stimulation in vitro.
    • Negative correlations were found between adipocyte insulin binding and fasting serum insulin/in vivo insulin resistance.

    Conclusions:

    • Both binding and post-binding defects in insulin target cells contribute to insulin resistance in hepatic cirrhosis.
    • Monocyte insulin binding is not correlated with in vivo insulin resistance in this patient group.
