Background:

  • Primary biliary cholangitis (PBC) is a chronic liver disease.
  • Ursodeoxycholic acid (UDCA) is a primary treatment for PBC.
  • Predicting treatment response and long-term outcomes is crucial for managing PBC patients.

Purpose of the Study:

  • To evaluate the utility of established prognostic scores in predicting biochemical response (BR) to UDCA.
  • To assess the ability of these scores to predict long-term liver-related complications in PBC patients.

Main Methods:

  • Retrospective single-center study of 50 female PBC patients on UDCA.
  • Biochemical response (BR) defined by Paris II criteria.
  • Calculation of APRI, ALBI, Mayo, GLOBE, and UK-PBC scores.
  • Median follow-up of 76 months for liver-related complications.

Main Results:

  • 64% of patients achieved BR to UDCA.
  • 40% of patients experienced liver-related complications during follow-up.
  • All evaluated scores demonstrated good prediction for BR (concordance statistics: 0.76-0.94).
  • Scores showed excellent negative predictive values for 5-year liver complications (concordance statistics: 0.73-0.89).

Conclusions:

  • Prognostic scores are effective tools for predicting UDCA biochemical response in PBC.
  • These scores accurately predict long-term liver-related complications in PBC patients.
  • Clinical implementation of these prognostic scores is recommended for improved patient management.
