Summary
Surgical orthodontic treatment for Vertical Maxillary Excess improved facial esthetics and function. Bimaxillary surgery successfully reduced lower facial height, correcting malocclusion and alleviating temporomandibular joint symptoms.

- Bimaxillary surgery, including LeFort I osteotomy and bilateral sagittal split osteotomy, was performed without extractions.
- Superior repositioning of the maxilla by 7-8 mm and mandibular derotation were achieved.
- Post-operative results included a 4° decrease in the mandibular plane angle, achievement of skeletal class I, correction of lip incompetence, and improved gingival display.

- This approach resolved TMJ clicking in the reported case.
