Intraoperative neuromonitoring (IONM) did not significantly reduce vocal cord palsy (VCP) rates after thyroidectomy. However, surgeon experience appears to decrease VCP risk over time.
Area of Science:
- Endocrinology
- Surgical Oncology
- Neurosurgery
Background:
- Thyroidectomy is a common procedure for thyroid cancer.
- Vocal cord palsy (VCP) is a potential complication.
- Intraoperative neuromonitoring (IONM) is used to minimize nerve damage.
Purpose of the Study:
- To evaluate the impact of IONM on VCP rates.
- To assess temporal trends in VCP following thyroidectomy.
Main Methods:
- Retrospective analysis of 712 patients undergoing thyroidectomy for thyroid cancer (2014-2022).
- Comparison of VCP rates between non-IONM and IONM groups.
- Analysis of risk factors and temporal trends in VCP.
Main Results:
- No significant difference in transient or permanent VCP rates between non-IONM and IONM groups.
- Transient VCP: 4.6% (non-IONM) vs. 4.3% (IONM).
- Permanent VCP: 0.7% (non-IONM) vs. 0.4% (IONM).
- A significant decreasing trend in VCP rates observed over time (p=0.017).
Conclusions:
- IONM did not significantly reduce VCP risk in this study.
- Increasing surgeon experience may be associated with lower VCP rates.
- Further research may explore the role of experience in VCP prevention.
