Summary

Ultrasound effectively detects unstable ramp lesions in anterior cruciate ligament (ACL) injuries. Stable lesions were harder to visualize, highlighting US utility for specific ACL complications.

Background:

  • Ramp lesions, often suspected via MRI or US, can heal without instability.
  • Intraoperative arthroscopy is the gold standard for assessing ramp lesion stability.

Purpose of the Study:

  • To evaluate the efficacy of ultrasound (US) in diagnosing ramp lesions preoperatively and intraoperatively.
  • To assess the detection rate and reliability of US for ramp lesions.

Main Methods:

  • Eighty-two knees undergoing anterior cruciate ligament (ACL) reconstruction were analyzed.
  • Ultrasound detection rates at initial and preoperative visits were compared to arthroscopic findings.
  • Test-retest reliability was assessed using intraclass correlation coefficient.

Main Results:

  • Ultrasound identified ramp lesions in 90.0% of cases initially; 22.2% were poorly delineated preoperatively.
  • Unstable ramp lesions were successfully delineated by US during surgery.
  • Stable ramp lesions proved difficult to visualize with US.

Conclusions:

  • Ultrasound can detect unstable ramp lesions associated with ACL injuries.
  • US utility is higher for unstable ramp lesions compared to stable ones.
