Background:

  • General practitioners (GPs) were crucial in managing the COVID-19 pandemic.
  • Identifying distinct clinical profiles in COVID-19 patients is essential for improving care and monitoring high-risk individuals.

Purpose of the Study:

  • To identify distinct clinical profiles of COVID-19 in adult primary care patients.
  • To determine if these identified profiles correlate with adverse outcomes, including hospital admission and persistent symptoms.

Main Methods:

  • A prospective multicentre study involving 340 adult COVID-19 patients recruited by 44 GPs in the Paris area.
  • Latent class analysis (LCA) was employed using 11 clinical signs and symptoms to define patient profiles.
  • Outcomes assessed included a 3-month composite of hospital admission or death, and persistent symptoms at 3 and 6 months post-diagnosis.

Main Results:

  • Six distinct COVID-19 patient profiles were identified: 'paucisymptomatic', 'anosmia/ageusia', 'influenza-like with anosmia/ageusia', 'influenza-like without anosmia/ageusia', 'influenza-like with respiratory impairment', and 'complete form'.
  • Patients in the 'influenza-like with respiratory impairment' profile showed higher rates of hospitalisation at 3 months.
  • Persistent symptoms at 3 months were more common in the 'influenza-like with respiratory impairment' and 'complete form' profiles.

Conclusions:

  • The identified COVID-19 profiles can assist GPs in recognizing patients at elevated risk for persistent symptoms and hospitalisation.
  • These findings support the implementation of targeted monitoring strategies for at-risk patient groups within primary care settings.
