Toe-to-hand reconstruction effectively restored function and appearance after malignant tumor removal, offering a cosmetic alternative to traditional finger transposition. This innovative approach enhances patient satisfaction and hand usability.

Area of Science:

  • Orthopedic Surgery
  • Reconstructive Microsurgery
  • Surgical Oncology

Background:

  • Malignant tumor resection in the hand often necessitates digit amputation, potentially compromising hand function and aesthetics.
  • Adjacent finger transposition is a common reconstructive technique but may result in patient dissatisfaction due to altered hand appearance.

Observation:

  • A 28-year-old male patient with a malignant fibroblastic tumor at the base of the ring finger underwent tumor resection, involving the phalanx and part of the metacarpal.
  • The patient declined conventional ray amputation and subsequent fifth-finger transposition due to cosmetic concerns.

Findings:

  • A novel reconstruction was performed using a long-vascularized subtotal second toe graft, extending from the metacarpal neck to the middle phalanx base of the fourth finger.

  • The patient experienced no tumor recurrence and reported high satisfaction with both hand function and cosmetic outcome at three years post-surgery.
  • Implications:

    • Vascularized toe-to-hand transplantation presents a viable and aesthetically superior alternative for complex hand defects following tumor resection.
    • This technique can significantly improve patient quality of life by restoring both functional and cosmetic aspects of the hand.
