Types of Intermediate Filaments01:31

Types of Intermediate Filaments

3.7K
The intermediate filaments are an essential component of the cytoskeleton. Presently six types of intermediate filament have been identified. Type I and II are acidic and basic keratin proteins. Type III is of mesodermal origin and comprises four proteins: vimentin, desmin, glial fibrillary acidic protein (GFAP), and peripherin. Vimentin is commonly found in mesenchymal cells, desmin in muscle cells, GFAP in astrocytes, while peripherin is found in peripheral nervous system neurons (PNS). Type...

Disassembly of Intermediate Filaments01:35

Disassembly of Intermediate Filaments

2.0K
Intermediate filaments (IFs) do not undergo spontaneous disassembly. Enzymes, kinases, and phosphatases add and remove phosphates from specific sites to regulate their disassembly. The IF concentration in the cytoplasm also regulates the disassembly. If the concentration crosses a threshold, it activates the protein kinases in the vicinity, allowing the phosphorylation of IFs.
Keratin proteins, found at the cell periphery near cell junctions, undergo a cycle of assembly and disassembly. In Type...

Area of Science:

  • Ophthalmology
  • Corneal Diseases
  • Dry Eye Disease

Background:

  • Filamentary keratitis (FK) presents as a clinical sign linked to ocular and systemic issues.
  • It causes significant ocular discomfort, including irritation, tearing, and photophobia.
  • FK is a debilitating condition often associated with dry eye, impacting patients' daily activities.

Purpose of the Study:

  • To review the pathogenesis and management of filamentary keratitis.
  • To discuss traditional and novel treatment strategies for FK.

Main Methods:

  • Literature review of filamentary keratitis.
  • Analysis of etiopathogenesis and clinical presentation.
  • Compilation of current and emerging treatment modalities.

Main Results:

  • FK involves sub-epithelial or Bowman's membrane dysfunction leading to filament formation.
  • Eyelid movement exacerbates filament elongation.
  • Management strategies encompass addressing underlying causes, lubricants, corticosteroids, mucolytics, bandage contact lenses, punctal plugs, and mechanical removal.

Conclusions:

  • Filamentary keratitis has multifactorial causes and pathogenesis.
  • Various treatments are available, with topical immunomodulators showing promise for dry eye-associated FK.
  • Prognosis is generally fair, though recurrences can occur.
