Prostatic Artery Embolization (PAE) is a safe and effective treatment for benign prostatic hyperplasia (BPH). This study shows PAE significantly improves urinary symptoms and quality of life with minimal complications.

Area of Science:

  • Interventional Radiology
  • Urology

Background:

  • Benign prostatic hyperplasia (BPH) is a common condition in aging men.
  • Lower urinary tract symptoms (LUTS) associated with BPH significantly impact quality of life.
  • Current treatments for BPH have potential side effects, necessitating exploration of minimally invasive options.

Purpose of the Study:

  • To evaluate the long-term efficacy and safety of Prostatic Artery Embolization (PAE) for BPH treatment.
  • To assess changes in prostatic volume, PSA levels, and patient-reported outcomes after PAE.
  • To analyze procedural improvements and complication rates over a five-year period.

Main Methods:

  • Retrospective analysis of 551 PAE procedures performed between January 2019 and July 2023.
  • Collection and analysis of patient demographics, procedural details (e.g., particle size), and complication data.

  • Assessment of pre- and post-PAE prostatic volume, PSA, IPSS, QoL, and IIEF scores, with statistical analysis.
  • Main Results:

    • PAE resulted in significant reductions in prostatic volume (p < .001) and PSA levels (p < .001) at three months post-procedure.
    • Patients experienced substantial improvements in IPSS (average -9 points) and QoL scores (average -2 points).
    • Procedural efficiency improved significantly, with reduced fluoroscopy and procedure times from 2019 to 2023 (p < .0001).
    • Only minor complications (n=16) were reported; no major complications occurred.

    Conclusions:

    • PAE is a safe and effective treatment option for men with BPH.
    • PAE offers significant relief from LUTS and improves quality of life.
    • Sexual function is maintained following PAE, making it a favorable choice for eligible patients.
