Inflammatory Bowel Disease V: Surgical Management

139
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Summary

Experts recommend elongating the biliopancreatic limb (BPL) for recurrent weight gain after one anastomosis gastric bypass (OAGB). Conversion to Roux-en-Y gastric bypass (RYGB) is also recommended for bile reflux, with hiatal hernia repair during conversion.

Area of Science:

  • Bariatric Surgery
  • Gastroenterology
  • Surgical Complications

Background:

  • Limited evidence exists for managing complications after one anastomosis gastric bypass (OAGB).
  • Suboptimal weight loss, weight regain, and worsening obesity complications require clear treatment guidelines.

Purpose of the Study:

  • To establish expert consensus on managing complications and revision surgeries following OAGB.
  • To provide clinical guidance for suboptimal outcomes after OAGB.

Main Methods:

  • A modified Delphi consensus method was employed.
  • Forty-eight bariatric surgeons from 28 countries participated in two voting rounds on 64 statements.
  • Consensus was defined as agreement among ≥70.0% of experts.

Main Results:

  • Consensus was reached on 46 statements.
  • Elongation of the biliopancreatic limb (BPL) is an acceptable option for recurrent weight gain or worsening complications post-OAGB.
  • Conversion to Roux-en-Y gastric bypass (RYGB), with or without pouch downsizing, is accepted for persistent bile reflux, with hiatal hernia repair recommended during conversion.

Conclusions:

  • Expert consensus was achieved on several aspects of OAGB revision and conversion surgeries.
  • Areas of disagreement persist, indicating a need for further research.
  • The findings provide valuable resources for clinicians managing OAGB complications.
