Background:

Schistosomiasis is a prevalent parasitic disease in developing nations.
Appendicitis caused by Schistosoma (schistosomal appendicitis) is rare, even in endemic regions.
This condition predominantly affects young males and is often an incidental finding during appendectomy.

Observation:

A 24-year-old male presented with abdominal pain and was diagnosed with acute appendicitis.
Appendectomy revealed inflammation and Schistosoma ova in the appendix histopathology.
Serology for Schistosoma was weakly positive; stool and urine analyses were negative.

Findings:

Histopathological examination confirmed schistosomal appendicitis.
The patient received praziquantel and had an uncomplicated 3-year post-operative follow-up.
Abdominal ultrasound showed no complications.

Implications:

Schistosomiasis should be considered in the differential diagnosis of appendicitis in endemic areas.
Histopathological evaluation is crucial for accurate diagnosis of schistosomal appendicitis.
