    Percutaneous aspiration of hepatic hydatid cysts, though risky, was successfully used to treat a recurrent case. This minimally invasive technique offers a potential alternative for patients unsuitable for surgery.

    Background:

    • Percutaneous aspiration has been historically avoided due to risks like anaphylaxis and cyst dissemination.

    Observation:

    • A patient with a recurrent hepatic hydatid cyst underwent percutaneous needle aspiration under radiologic guidance.
    • The cyst was subsequently lavaged using silver nitrate and hypertonic saline via a catheter.

    Findings:

    • The percutaneous procedure was successful in treating the recurrent hepatic hydatid cyst.
    • One year post-procedure, the patient maintained normal liver function test results.

    Implications:

    • Percutaneous catheter drainage presents a viable alternative treatment for hepatic Echinococcus disease, especially for non-surgical candidates.
    • This approach may reduce the morbidity associated with traditional surgical management.
