Summary

Complement fragment C4d is expressed in Focal Segmental Glomerulosclerosis (FSGS) subtypes, particularly cellular and collapsing types. C4d positivity correlates with increased proteinuria and may serve as a marker in FSGS.

Background:

  • Limited data exists on C4d complement fragment expression in Focal Segmental Glomerulosclerosis (FSGS) subtypes.
  • Understanding C4d's role in FSGS pathogenesis is crucial for potential diagnostic and prognostic applications.

Purpose of the Study:

  • To investigate C4d expression in various FSGS subtypes using percutaneous native renal biopsies.
  • To correlate C4d expression with clinical, biochemical, and histological parameters in FSGS patients.

Main Methods:

  • Retrospective analysis of 20 FSGS patient renal biopsies (aged 16-65, non-diabetic, non-obese).
  • Immunohistochemistry performed for C4d detection in glomerular capillaries (GC) and sclerosis areas (SA).
  • Comparison of C4d positive vs. negative groups; correlation analysis with clinical and histological data.

Main Results:

  • C4d expression was observed in 40% of GC, 30% of SA, and 35% of mesangium.
  • Highest C4d expression noted in cellular and collapsing FSGS subtypes.
  • C4d positivity associated with increased proteinuria (p=0.035) and correlated with SA and tubulointerstitial damage (p<0.05).

Conclusions:

  • C4d expression in FSGS, especially cellular and collapsing subtypes, indicates complement activation.
  • C4d may function as a potential surrogate marker in FSGS diagnosis and management.
