Background:

  • Functional disability is a growing concern in aging populations worldwide.
  • Masticatory function's role in functional disability is unclear, especially concerning the influence of frailty.

Purpose of the Study:

  • To investigate the association between masticatory function and incident functional disability in older adults.
  • To determine if frailty mediates the relationship between masticatory function and functional disability.

Main Methods:

  • A cohort of 40,562 community-dwelling older adults (≥65 years) was followed for a median of 3.0 years.
  • Masticatory function was self-reported as good, moderate, or poor.
  • Functional disability (need for long-term care certification) was the primary outcome, analyzed using Cox proportional hazards models.

Main Results:

  • Poor and moderate masticatory function were significantly associated with higher risk of functional disability.
  • After adjusting for frailty indicators (underweight, exercise, gait speed), this association was attenuated.
  • Hazard ratios for functional disability decreased notably in moderate (1.21 to 1.06) and poor (1.64 to 1.51) masticatory function groups.

Conclusions:

  • Masticatory dysfunction is significantly associated with incident functional disability in older Japanese adults.
  • The findings suggest masticatory dysfunction may act as a surrogate marker for frailty.
  • Interventions targeting frailty may be more effective than those solely focused on masticatory function for preventing disability.
