    Background:

    • Growing demand for skin quality interventions in aesthetic medicine necessitates objective assessment tools.
    • Existing methods may lack the standardization required for evaluating novel treatments or devices.

    Purpose of the Study:

    • To develop and validate a 5-point photonumeric rating scale for assessing overall skin quality.
    • The scale evaluates key attributes: radiance, color evenness, and smoothness.

    Main Methods:

    • The IBSA (Institut Biochimique SA) Composite Skin Quality Scale was developed and validated with 88 diverse patients.
    • Board-certified dermatologists and plastic surgeons conducted two rounds of assessments.
    • Reliability was measured using weighted kappa and intraclass correlation coefficient (ICC).

    Main Results:

    • High intrarater reliability was observed (kappa: 0.812-0.815, ICC: 0.903).
    • Interrater reliability demonstrated substantial to almost perfect agreement (kappa: 0.654-0.853, ICC: 0.657-0.855).
    • The scale effectively detected clinically relevant 1-grade differences in skin quality.

    Conclusions:

    • The IBSA Composite Skin Quality Scale is a clinically relevant and highly reliable tool.
    • It is suitable for objective skin assessment in clinical studies.
    • The scale supports the evaluation of new aesthetic technologies and products.
