Risk Controls Identified in Action Plans Following Serious Incident Investigations in Secondary Care: A Qualitative

Serious incident investigations often fail to address all contributing factors, with most recommended risk controls being weak and individualized. This limits the translation of investigations into effective systemic patient safety improvements.
