    Background:

    • The endometrium, the inner lining of the uterus, plays a crucial role in reproduction.
    • Understanding the immune components within the endometrium is vital for reproductive health.
    • Previous research has indicated the presence of immune cells in the endometrium, but their organization and characteristics require further elucidation.

    Purpose of the Study:

    • To characterize the histological and immunological features of endometrial lymphoid tissue.
    • To compare endometrial lymphoid tissue with mucosal-associated lymphoid tissue (MALT) found in other body sites.
    • To investigate the distribution and cellular composition of lymphoid cells within the endometrium.

    Main Methods:

    • Histological, immunohistological, and electron microscopy techniques were employed.
    • A panel of monoclonal antibodies was used to identify macrophages (OKMI), HLA-DR antigen, B lymphocytes, T lymphocytes, and dendritic reticulum cells.
    • A two-stage indirect immunoperoxidase labeling technique was utilized for cell identification.

    Main Results:

    • Endometrial lymphoid tissue exhibited a consistent pattern across all analyzed uteri.
    • Lymphoid tissue was identified intraepithelially, interstitially, and in lymphoid aggregates within the stratum basalis.
    • Intraepithelial lymphocytes were predominantly T lymphocytes, associated with HLA-DR+ and OKMI+ macrophages. Interstitial areas contained lymphocytes and macrophages. Aggregates were mainly T lymphocytes with some B lymphocytes.
    • Dendritic reticulum cells were observed in lymphoid aggregates with germinal centers.

    Conclusions:

    • Endometrial lymphoid tissue shares characteristics with mucosal-associated lymphoid tissue (MALT) found in the bronchus and intestine.
    • The presence of T lymphocytes, macrophages, and dendritic cells suggests an immune surveillance role.
    • Endometrial lymphoid tissue is unique due to its location within the stratum functionalis, which undergoes cyclical shedding during the menstrual cycle.
