    Background:

    • Essential hypertension is a common condition without target organ damage in many patients.
    • Understanding the effects of antihypertensive medications is crucial for effective treatment.

    Purpose of the Study:

    • To evaluate the effects of single and multiple doses of nicardipine on urine output, blood pressure, and related parameters in patients with essential hypertension.
    • To assess the impact of nicardipine on plasma volume and plasma renin activity.

    Main Methods:

    • Seven untreated patients with essential hypertension were studied.
    • Urine output, intra-arterial blood pressure (IABP), plasma volume, and plasma renin activity (PRA) were measured.
    • A single 30 mg oral dose of nicardipine was administered, followed by a 2-month outpatient treatment with varying doses (20-40 mg four times daily).

    Main Results:

    • Single and multiple doses of nicardipine significantly increased urine output between 08:00 and 13:00 h.
    • Mean IABP was significantly reduced after both single and multiple doses of nicardipine.
    • No significant changes were observed in plasma volume, weight, or PRA during the study.

    Conclusions:

    • Nicardipine effectively reduces blood pressure in patients with essential hypertension.
    • The drug also influences renal function, indicated by increased urine output and natriuresis after a single dose.
    • Long-term nicardipine treatment leads to sustained blood pressure reduction without significant alterations in plasma volume or renin activity.
