Summary

This case report details the surgical repair of a right coronary sinus aneurysm using a Dacron graft and coronary reimplantation. This successful intervention ensured a favorable outcome for the patient with sinus of Valsalva aneurysm.

Background:

  • Aneurysms of the sinus of Valsalva, particularly involving the right coronary cusp, are rare but can lead to significant complications.
  • Surgical intervention is indicated based on current aortic guidelines for managing such conditions.

Observation:

  • The patient underwent surgical treatment via median sternotomy and cardiopulmonary bypass.
  • Intraoperative visualization confirmed the aneurysm's origin from the right coronary sinus, adjacent to the right coronary artery ostium.
  • The aneurysm sac was found to be in close proximity to the fragile aortic tissue beneath the coronary ostium.

Findings:

  • Surgical excision of the affected right coronary button and sinus wall was performed.
  • Reconstruction of the right coronary sinus was achieved using a Dacron graft.
  • Successful reimplantation of the right coronary artery was completed, with an uneventful postoperative course and discharge on day 7.

Implications:

  • Complete sinus reconstruction is a viable and preferred surgical strategy over local patching for right coronary sinus aneurysms, especially when near the coronary ostium.
  • This approach minimizes risks associated with coronary artery compromise and ensures structural integrity of the aortic root.
  • The successful surgical management highlights the effectiveness of graft reconstruction in treating complex aortic sinus pathologies.
